"1. Observed employee wearing gloves place raw bacon on grill, remove gloves, done new gloves, then handle clean plates and cooked food along front food service grill area. 2. Observed employee remove gloves, pull pants up on waist, done new gloves, then touch clean equipment and handle cooked food along front food service grill area. C/A: Food employees shall clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and: (i) After touching bare human body parts other than clean hands and clean, exposed arms; (ii) After using the toilet room; (iii) After caring for or handling service animals or aquatic animals; (iv) After coughing, sneezing, using a handkerchief or disposable tissue, using tobacco products or electronic devices that simulate tobacco smoking, eating or drinking, except for drinking from a closed beverage container and the container is handled to prevent contamination of the hands; (v) After handling soiled equipment or utensils; (vi) During food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing tasks; (vii) When switching between working with raw food and working with ready-to-eat food; (viii) Before donning gloves to initiate a task that involves working with food; and (ix) After engaging in other activities that contaminate the hands. COS: Equipment cleaned, food replaced, employee washed hands."

"Observed staff using the dishwasher without the dishwasher reaching the final rinse sanitizing temperature of 180*F. Sticker remained black after testing the dishwasher. C/A Mechanical Warewashing Equipment, Hot Water Sanitization Temperatures. 1. Except as specified in paragraph 2 of this subsection, in a mechanical operation, the temperature of the fresh hot water sanitizing rinse as it enters the manifold may not be more than 194ºF (90ºC), or less than: Pf (i) For a stationary rack, single temperature machine, 165ºF (74ºC); Pf or (ii) For all other machines, 180ºF (82ºC). Pf COS Manager was instructed to direct staff to wash dishes at the four compartment sink until the dishwasher is repaired. Manager will need to submit a repair invoice to EHS within 72 hours stating the dishwasher has been repaired Update: The manager submitted a picture with the sticker turned orange, the dishwasher needed to have the booster heater reset. | Observed the ice bin located in the service area with mold build-up on the edge of the chute. C/A Cleaning of Equipment and Utensils. (a) Equipment, Food-Contact Surfaces, and Utensils. 1. Equipment food-contact surfaces and utensils shall be clean to sight and touch. Pf. COS Manager cleaned the ice bin before EHS left the premises"

CA Wiping cloths must be held between uses in a chemical sanitizer solution at a concentration specified under DPH Rule 511-6-1-.05(6) COS Manager placed them in the sanitizing solution | Observed no inspection posted. C/A Must ensure inspection report is always posted for public display to allow customers to review the report. MUST POST THE CURRENT INSPECTION REPORT WITHIN 72 HOURS"
