"Observed TCS food that was held at the prep line with a temperature above 41*F. C/A Time/Temperature Control for Safety Food, Hot and Cold Holding. Except during preparation, cooking, or cooling, or when time is used as the public health control, time/temperature control for safety food shall be maintained at 41°F (5°C) or below or 135°F (57°C) or above, except that roasts cooked to a temperature and for a time specified in subsection (5)(a)2 of this Rule and reheated using the same temperature and time conditions as cooking may be held at a temperature of 130°F (54°C) or above. P COS Manager moved the TCS items into the cooler to cool off because the food was within the 4 hour window | Observed one employee at the prep area with a gold bracelet around his wrist. Observed another employee with a large gold wrist watch on the wrist in the main kitchen at the prep area. C/A Jewelry. Except for a plain ring such as a wedding band food employees may not wear jewelry including medical information jewelry on their arms and hands while preparing food. COS Manager had the employees remove their jewelry"

"Ensure designated person(s)- in-charge are aware of the food safety responsibilities, which includes responding to health authority’s concerns about food safety violations. Upon arrival onsite, EHS introduced herself to PIC. After observed hot/cold TCS food in the first kitchen prep cooler and prep top warmer, EHS informed PIC of temperature violations and the necessity of discarding food. requested assistance more than (3) times before another employee assisted with discarding of cold TCS food with temperatures above 41*f. Employee was talking on his mobile and slow to respond and using (1) hand to remove food. EHS began removing and storing on countertop larger quantities of food. After 20 - 30 minutes, upper level management began arriving. Food was discarded. | Observed Quat sanitizer solution buckets X3 and 3 compartment sink tested below 150 ppm. CA: Maintain proper chemical concentration for sanitization. Quat sanitizer solution shall be maintained at concentration of 150-400 ppm as per chemical manufacturer's label. PIC provided fresh Quat sanitizer at 400ppm"
